• Home Care Packages
• Team Care Arrangement - talk to your GP
• TAC (Transport Accident Commission)
• NDIS (National Disability Insurance Scheme)
• Self-funded

What to expect

Most people attend Nexus for their appointment. We might be able to see you in your home if you can't get to us and live in:

• Mitchell Shire
• Murrindindi Shire
• Southern part of the Strathbogie Shire

You will have an individual assessment. We will write a Care Plan. This Plan is based on your needs.

We will talk about your goals. We can give you information to help you achieve your goals and manage your condition.

You can have a follow up appointment so we can keep helping you.
