NDIS

Nexus Primary Health is a NDIS Registered Service Provider. We will work with you to help you find services to achieve the goals in your NDIS plan.

You will need to have a current approved plan to use NDIS services at Nexus.

We do Support Coordination. Support Coordination is part of the Capacity Building Budget. This is to build skills and independence. We will work with you so that you can get the most out of you NDIS funds.  The Support Coordinator can help you with:
  • Your Plan
  • Budgets
  • Service agreements.
  • Getting the right supports
  • Working with allied health professionals
  • Help you get ready for a plan review.
  • Get the most out of your NDIS funds
  • Understand how the NDIS works

What to expect

We will call you after we have your referral. Our Intake Team will talk to you about the services you would like. We will also talk about where you would like those services to be delivered.

We will be able to help deliver the services you need after we have a talk about it.
