Live Well – Choose the Life!

Every 12 minutes in Australia someone is diagnosed with Diabetes or Heart Disease tragically effecting 280 people each day.

If you 18 or over and are at risk of Diabetes, Heart Disease or Stroke then Nexus has the program for you! Over 55,000 Australians completed the Life! Program – greatly improving their health, allowing them to live fuller and more satisfying lives.

Choose the Life! Program @ Nexus – it is FREE!

Risk factors include having Prediabetes, history of Gestational Diabetes, Polycystic Ovarian Syndrome, Heart Disease, High Cholesterol and Blood Pressure and being overweight.

Over six group sessions, Life! will teach you how to adopt sustainable and healthy behaviours to reduce your risk of these three serious conditions.
Your first session is a one on one with a Nexus Dietitian to introduce you to the program and work with you to set some personal health goals.  Followed by fortnightly interactive and fun group sessions at the Nexus GP SuperClinic in Wallan.

The lifestyle changes you learn through the Life! course will not improve your wellbeing, it will also help you manage and prevent other medical problems like high cholesterol. We understand that changing your lifestyle isn’t easy on your own – that’s why we’re here to help.

Strengthening community health: Nexus Primary Health and Sunbury and Cobaw Community Health to explore a merger

Nexus Primary Health (Nexus) and Sunbury and Cobaw Community Health (SCCH) are excited to share the first step to explore the creation of a stronger, more resilient community health service for people who live in Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas. On Monday 13 January 2025, Nexus transitioned to a governance structure led by SCCH. While both organisations remain separate entities, they will be governed by the same board, and Nexus’ Chief Executive Officer will report to SCCH’s Chief Executive Officer, allowing the organisations to better explore the potential community benefits of a merger.

Commonwealth Home Support Programme (CHSP) contract changes in 2025

From Wednesday, 1 January 2025, the Commonwealth Home Support Programme (CHSP) contract for the Mitchell, Strathbogie, and Murrindindi local government areas will transfer to Sunbury & Cobaw Community Health (SCCH).  While SCCH will manage the contract, Nexus Primary Health will continue delivering most CHSP services through a subcontract model.
