Commonwealth Home Support Programme (CHSP) contract changes in 2025

At Nexus Primary Health we believe collaboration and partnerships are the key to achieving our vision of living well in your community. Given the increased pressure across the health sector, it’s more important than ever for organisations to leverage partnerships to uphold continuity of care for clients.


From Wednesday, 1 January 2025, the Commonwealth Home Support Programme (CHSP) contract for the Mitchell, Strathbogie, and Murrindindi local government areas will transfer to Sunbury & Cobaw Community Health (SCCH).  While SCCH will manage the contract, Nexus Primary Health will continue delivering most CHSP services through a subcontract model.


SCCH and Nexus share a commitment to providing accessible, comprehensive health and community services to the community. This arrangement will enable the Nexus team to continue delivering high-quality services to clients across the Mitchell, Strathbogie and Murrindindi local government areas.
Our organisations have a strong history of collaboration. Partnering with SCCH is a strategic step to ensure uninterrupted service delivery for clients, while supporting both organisation’s long-term sustainability.


We are looking forward to working closely with SCCH to continue delivering accessible care to clients in their local communities.

Strengthening community health: Nexus Primary Health and Sunbury and Cobaw Community Health to explore a merger

Nexus Primary Health (Nexus) and Sunbury and Cobaw Community Health (SCCH) are excited to share the first step to explore the creation of a stronger, more resilient community health service for people who live in Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas. On Monday 13 January 2025, Nexus transitioned to a governance structure led by SCCH. While both organisations remain separate entities, they will be governed by the same board, and Nexus’ Chief Executive Officer will report to SCCH’s Chief Executive Officer, allowing the organisations to better explore the potential community benefits of a merger.
